General Description

Meets agency Mission by overseeing the success of the agency’s community-based programs within Clatsop or Tillamook County. Such programs include: nutrition services, Family Caregiver Support (FCSP), Family Caregiver Assistance Program (FCAP), Senior Peer Mentor Program (SPMP), Senior Health Insurance Benefits Assistance (SHIBA), options counseling (OC), ERC—a self-empowered training program for individuals who are people with disabilities or seniors who employ homecare workers, the Otago fall-prevention program, and other healthy-aging, disease-prevention, and health-promotion programs.

Essential Functions

  1. Develops and promotes new and ongoing program services in assigned county, including outreach and volunteer recruitment, to ensure program success
  2. Oversees program services in assigned county
  3. Ensures assigned staff and program volunteers meet agency mission, values, and program requirements
  4. Reduces client and agency risk
  5. Provides options counseling services and private-admission assessments (PAA)
  6. Connects family caregivers with resources and services
  7. Promotes Aging and Disability Resource Connection (ADRC) concept, services, and specifically options counseling and family caregiver services
  8. Provides excellent customer service in a professional manner

1. Develops and promotes new and ongoing program services in assigned county, including outreach and volunteer recruitment, to ensure program success.

  • evaluate needs and identify program opportunities
  • develop and implement outreach activities to seek out recipients for all services
  • educate the community
  • network with community partners

2. Oversees program services in assigned county by:

  • work closely with community programs manager, program managers and other managers to ensure services within assigned area are meeting client needs, quality standards, federal, state and agency requirements
  • recommend changes as needed
  • screen and process new referrals to the HOPE Senior Peer Mentor Program, conduct interviews and provide program overview to consumers, conduct home visits, administer PHQ-4 screening tools, and document the visit and disposition according to program policy and procedure
  • conduct intake appointments (via phone or home visit) for the Otago fall-prevention program, coordinate weekly Otago team meetings, and act as liaison between NWSDS staff and Otago program staff on shared consumers.

3. Ensures assigned staff and program volunteers meet agency mission, values and program requirements

  • manage some volunteers directly and others indirectly through staff
  • manage, coach, and mentor staff and volunteers
  • recruit, train, and manage performance by effectively applying human resource policies
  • promote two-way communication, engage others, listen, provide feedback and express appreciation for staff and volunteers
  • provide direct services as necessary when staff or volunteers are not available to ensure program functionality

4. Reduces client and agency risk

  • embrace and exhibit agency mission and core values, and provide excellent customer service
  • exhibit good decision making, problem solving, and confidence in management skills
  • serve as a mandatory reporter of suspected abuse of vulnerable populations as required by policy and regulation
  • maintain and share information according to privacy regulations
  • comply with ethical and professional standards
  • ensure assigned staff and volunteers meet these requirements

5. Provides Options Counseling services and Private Admission Assessments (PAA)

  • determine the need and appropriateness of options counseling through gathering information from seniors or people with disabilities, family members, professionals, and other support resources about the individual’s wants, preferences, goals, and willingness to participate
  • explain available options, facilitate decision making, support and advocate for individual’s preferences and decisions, and develop action plan
  • facilitate access to services by helping the individual and family initiate contacts for services, assist with application process, make calls, follow up on progress, and evaluate situations throughout the process, identify changes and alter care plan.
  • Analyze available care options based on the assessment conducted.

6. Connects family caregivers with resources and services

  • identify and assess potential caregiver needs, provide care planning consultation and intervention in gaining access to services, assist with application process
  • determine eligibility for Family Caregiver Support Program or Family Caregiver Assistance Program services available based on resources and situation
  • locate services, conduct assessments, monitor services provided, work with caregivers to ensure access to services
  • facilitate family caregiver support groups
  • approve respite care services and program payments according to authority guidelines

7. Promotes Aging and Disability Resource Connection (ADRC) concept and services, and specifically promotes Options Counseling and Family Caregiver Services

  • educate self and others of private and public resources available for populations served
  • consult with colleagues, community partners, and find electronic and written materials about resources and current issues and trends
  • share educational information obtained, provide updates for ADRC resource database, build relationships with community partners, and participate in networking
  • identify unpaid family caregivers and others for potential services through outreach with agency staff, community organizations, etc., including via community presentations

8. Provide excellent customer service in a professional manner

  • apply the required knowledge and skills

  • work independently, seek and offer assistance when needed

Job-Specific Skills Needed to Meet Agency Expectations

  • Demonstrate knowledge of program eligibility requirements, general knowledge of the dynamics of care giving and resources available and the Family Caregiver Support Program, and the work of the ADRC
  • Work with Seniors and People with Disabilities and Understand the Issues These Populations Experience (Social, Physical, Care, etc.) 

General Skills Needed to Meet Agency Expectations

  • Good research, writing and general communication skills. 
  • Good interpersonal skills, interacting in respectful and culturally appropriate way.
  • Speak, read, write and understand English; and follow verbal and written instruction.
  • Above average computer skills including ability to learn and utilize different software, database programs, format spreadsheets; and the ability to meet requirements of this outcome.

Warrenton (or other assigned) County Nutrition Site Coordinators and program volunteers.


Experience and Skills:

Minimum Qualifications - Education and Experience

A qualified applicant will have a minimum of three (3) years of experience in social services, working in a similar capacity with vulnerable populations.  All experience and education which demonstrates the knowledge, skills, and abilities required to perform the job will be considered; however, the following is preferred:

  • Bachelor’s degree in social sciences or related field
  • Experience supervising staff and volunteers

Work Environment and Physical Demands

Contact with the public in home or office environments may risk exposure to people with contagious diseases or irrational/hostile behavior and contact with domestic animals.                     

The physical demands are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

The essential functions of this job require it to be performed in the office and in the field, with the possibility of partial remote work schedule.

Field work requires frequent travel to community sites and agency offices. Field work requires driving an Agency car or employee car, carrying and using a laptop computer.  Office work will be conducted using general office equipment, includes substantial sitting and is performed primarily in a cubicle environment, with noise and interruptions, being on phones and working at a computer. 

These essential outcomes require regular sitting, talking, hearing, computer use; frequently required to use repetitive hand motion, handle or feel, and to stand, walk, reach, bend or lift up to 25 pounds.

Other Requirements

To be successful, candidates must:

  • Secure and maintain a valid Oregon driver’s license or have an acceptable alternative means of transportation
  • Attend work regularly to meet the demands of this job and to provide necessary services
  • Complete and pass a criminal background check
